You can fill out the form online once you register for your School Café account. To register for a School Cafe account, click here. If you prefer to print out the form and fill it out by hand, you may do so using the PDFs below and sending it to the school using the form or email below.

Please print out the PDF and using PEN (only), fill out the form completely. Once the form has been completed and signed, upload the form and submit it using the form below or email to vanessa_baxley@iacafl.org.
